Vehicle Owner Authorization Form

TheAutomaker/NASTF Security Registry is an online/encrypted system. When it is notpossible to get a digital signature at the time of service due to connectivityissues or when a Vehicle Security Professional is performing onsite work for arepair shop as a third party. VehicleSecurity Professionals are required to use this form to collect a manualsignature from the vehicle owner or their authorized agent and upload a pictureof the authorizing party’s driver license after completing the online D1. This willbe securely encrypted and only used in the event of a theft or law enforcementinquiry. Once completed these documents will only be available to SecurityRegistry Administrators.
